CORRECTIONS
I have tried as much as possible to transcribe this document as originally written. Since I cannot reasonably do this to the letter in all cases, I have decided to allow certain differences including the changing of original page numbers to fit the size of the font I am using, to not use the same font that was used, to put a space between each paragraph, to use only one space after periods, colons, and semicolons, to ignore extra spaces that were possibly made to allow justification of the text and to put them in where justification may be required for the digital copy, and to make corrections in certain cases which I have noted below.
With regard to the corrections below, I am using the page numbers in this digital copy and not the original. I will insert the original page numbers in brackets to make it easier to research back and forth from this digital copy and the original pages which I have included as JPEG images and PDF files.
